About the First Appointment

New patients will be sent forms to be read and returned electronically. Forms sent include a Notice of Privacy Practiccs (HIPAA) document, an Intake and Consent form, a Credit Card Authorization form and a Authorization to Release Information if you are currently seeing a therapist.

If you have engaged in psychiatric care in the past, please bring available past records or fax them before your first appointment to 844-344-8859. 

When you arrive to the office, please flip the switch by the name plate, Emily Bost-Baxter.

Fees

Payment is due by cash, check or credit card at each appointment. 

If an appointment is missed or cancelled without 24 hours notice, you will be billed for the full amount of the appointment.   

Insurance

Dr. Bost is not a participating provider for any insurance companies but she is happy to provide a "super bill" which you may submit to your insurance for reimbursement.  

Parking

Parking is available on the street or in a garage under the building for a fee. 

Covid-19/Coronavirus
